On the actx.com home page, select Sign In. Then select Sign in as a guest medical provider (under the password box).
You will then need to enter:

• A passphrase, which will be provided to you by the patient
• Your first name
• Your last name
• Your email address
• Only required on initial sign in
• NPI or License number
• Only required on initial sign in
• State is not required with your NPI number

Your email address and NPI or Medical License number are only required on initial sign in for each patient. Subsequent sign ins only need your name and the passphrase.
Your passphrase is valid for 1 year. If you have difficulty signing in, please contact help@actx.com for assistance.
If you have lost or forgotten your passphrase, please contact your patient since our help desk will not have it. Your patient will be able to recover the original passphrase, or generate a new one for you.
